Benefits New Hire Enrollment Forms

Classified/Management: Benefits begin the first of the month following your date of hire. If your start date is the first of the month, your benefits will also begin on that date.

Faculty:  Most instructors' benefits will begin the first of the month of their semester start date (August 1st or January 1st).  Some exceptions apply.

New Hire Benefits Forms: (Required*)

Medical Plan Options:  Medical Plan Comparison with Rates (effective 10/01/2025)

Select your medical plan and complete the appropriate form:

2.1*) Blue Shield: SISC Blue Shield Enrollment Form
       Premium Only Plan (POP) Form - (required for 80%-C, 90%-G, or 100%-D plan choices)
OR

2.2*) Kaiser: Kaiser Enrollment   

Important note for Kaiser Enrollees: You must live or work in an approved Kaiser Service Area.      Columbia Community College employees are not eligible unless they live in one of the listed service areas. To see if you qualify,

5*) Basic Life Beneficiary/Enrollment Form-Requires Original
