When To Contact The Benefits Office
Contact the Benefits Office if any of these changes occur:
- Marriage or Divorce (must be within 31 days from date of event) *
- Add a dependent due to birth, adoption, legal guardianship or full-time student status (must be within 31 days from date of event) *
- Remove overage dependent between ages 19 to 25 due to no longer a full time student
- Address change
*Failure to notify the Benefits Office within 31 days from date of event, your dependent will not be eligible for enrollment on your benefits until the District's Open Enrollment period held once a year. Coverage for members added during Open Enrollment will be effective January 1st of the following year.

- Any work-related injury/illness must be reported to your Supervisor immediately.
- Supervisor will complete and provide necessary paperwork for claim processing.
- Medical services must be obtained at the contracted facility - Occupational Medicine, 600 Coffee Rd., Modesto, CA unless a Pre-Designated Physician form has been turned into the Benefits Office prior to the injury/illness (this allows you to see your own personal physician).
